Affiliate membership is open to voluntary and non-profit organisations based in Longparish and wishing to support the aims and activities of the Community Association. Please fill in this form and return it to the Membership Secretary.

Notes

1. Please notify Longparish Community Association if you change your name and address
2. You can cancel the Gift Aid declaration at any time by notifying Longparish Community Association. The declaration will then not apply to donations you make on or after the date of cancellation, or from any date you specify.
3. You must pay an amount of income tax and/or capital gains tax at least equal to the tax that Longparish Community Association reclaims on your donation in the tax year (currently 28p for each £1 you give)
4. If your circumstances change and you no longer pay tax equal to the tax that Longparish Community Association reclaims, you must cancel your Gift Aid declaration by informing us.
5. If you pay tax at the higher rate you can claim further tax relief in your Self Assessment tax return.
6. If you are unsure about your donation qualifying for Gift Aid tax relief, ask Longparish Community Association or ask you local tax office for leaflet IR 113 Gift Aid.
